Put your main concerns first

1. Previous prescriptions

Bring prescriptions from earlier consultations, including those from other doctors. They can provide useful background about your treatment history.

2. Recent test reports

Carry any relevant blood test reports, scan reports, and other medical records you already have. Arrange them by date so they are easy to find. This checklist does not mean you need to get new tests before your appointment.

3. A list of current medicines

Write down the names and doses of the medicines you take, including over-the-counter products, vitamins, and supplements. If you are unsure of the names, bring the packaging or clear photographs of the labels. Include any known medicine allergies.

4. Notes about your symptoms

Note what has been bothering you, when it started, how often it happens, and anything you have noticed that makes it better or worse. A short note on your phone or a piece of paper is enough.

5. Questions for your doctor

Write down your main questions and put the most important ones first. You might ask what the next steps are, how to follow any instructions, or when to return for a follow-up. Ask for clarification whenever something is unclear.
